About the Role
The Sr. Associate, Strategic Engagement will play a key role within the Growth team and will be responsible for improving patient engagement and fostering participation in our care model. This position is dedicated to conducting patient outreach (telephonic and in-person home visits) in Western North Carolina.
What You’ll Do
- Patient Outreach & Engagement:
- Connect with prospective, high-priority patients and engage them in Hopscotch’s unique primary care model. Guide them to appropriate services at Hopscotchor help them find alternative primary care options.
- Community-Based Engagement:
- Spend approximately 50% of your time engaging with patients in community settings, including their homes, hospitals, long-term care facilities, and skilled nursing facilities. Reliable transportation is a requirement for this role.
- Documentation & HIPAA Compliance:
- Document interactions with patients promptly while maintaining HIPAA standards and confidentiality of protected health information.
- Communication & Coordination:
- Communicate effectively with patients, their families, caregivers, and healthcare providers. Ensure all parties involved in the patient’s care are well-informed and aligned on goals.
About You
A great candidate for the role has:
- Proven success in healthcare-related engagement, sales, or marketing roles.
- A problem-solving orientation and a flexible and positive attitude
- Strong people skills and an ability to communicate and build trust easily with Medicare patients
- Sales background preferred but not required
- Proficient in technology, including Microsoft Excel and EMR platforms.
- Ability to work independently in a fast-paced environment while effectively managing multiple priorities.
- Familiarity with Medicare and Medicare Advantage programs and processes is a plus
- Associate’s degree preferred, or equivalent education and work experience
- A personal vehicle (required), and the ability to travel throughout Western North Carolina to meet with patients.
