ABOUT THE ROLE

At Hopscotch Primary Care, we take a team approach to serve patient needs and provide the best care possible. Our goal is to provide the care each of us would want for ourselves or for our family members, in the right setting, and at the right time. The Hopscotch Care Team is responsible for delivering high-touch, high-quality care to patients, including coordination of care across different physicians and the broader healthcare continuum.

As Manager, Population Health, you will play a critical role in designing and leading the implementation of our population health operating model. The role will support core population health domains including but not limited to workflow optimizations, assessing vendor support and other tools, identifying and solving performance opportunities, and driving excellence across our population. This role will report to the Population Health Director.

WHAT YOU’LL DO

Core Responsibilities:

  • Assess, refine and develop workflow optimizations to enable best-in-class clinical outcomes for the Hopscotch model using a data-driven approach.
  • Collaborates closely with a cross functional team to build out the Hopscotch population health capabilities that contribute to a best-in-class Hopscotch care model for rural populations
  • Develop solutions that scale across multiple locations
  • Conduct independent ad hoc analysis to identify opportunities or troubleshoot issues
  • Review weekly and monthly KPIs and identify key performance improvement opportunities and drivers of metrics.
  • Ensure resources are allocated efficiently to reduce unnecessary utilization and control cost of care.
  • Play a key role in the assessment of accurate clinical documentation opportunities across the population.
  • Build and implement care model elements that meet all payer partner requirements.
  • Utilize data analytics to stratify patient populations and anticipate healthcare needs.
  • Develop robust insights from various sources, grounded in data and objective analysis, with the ability to make concrete recommendations and outline clear action steps to enable results

ABOUT YOU

We are looking for a motivated, mission driven, and experienced leader with:

  • 3-5 years of relevant experience in healthcare, population health, consulting, or other similar experience with strong preference for value-based care models
  • Bachelor’s Degree required
  • Graduate Degree a plus (MBA, MPH)
  • Ability to analyze data to build out a business case or ROI analysis
  • Strong analytical thinker with proficiency analyzing data and identifying insights
  • Experience working with multiple software & data platforms, electronic health records, spreadsheet development and navigation, data processing, organizational skills and ability to manage multiple priorities appropriately
  • US work authorization
  • Ability to travel as needed up to 40%
